How Medicines Are Priced in Greece: A Practical Guide

Pricing a medicine in Greece is not a negotiation in the usual sense — it is largely a calculation, driven by the prices of other European markets and published in an official bulletin. Understanding the mechanics is essential to any launch plan.

Greece uses external reference pricing: the price of an on-patent originator medicine is set by reference to the lowest prices across a basket of EU Member States. Because the reference basket and methodology have changed across successive reforms, the exact parameters should always be confirmed against the current rules before modelling.

The price bulletin

Prices are enacted through the price bulletin (Δελτίο Τιμών) issued by EOF, the National Organisation for Medicines. Re-pricing happens periodically — broadly twice a year — publishing maximum ex-factory, wholesale and retail prices. A product’s commercial reality can shift with each bulletin.

Generics and off-patent products

Generics are priced as a defined percentage of the reference off-patent price. When an originator loses exclusivity, its maximum price is cut to the lower of a set percentage of its last on-patent price or the average of the lowest EU prices. Greece has historically struggled with low generic uptake, a structural feature that shapes the whole market.

Clawback and rebates

Price is only half the story. Greece operates a closed pharmaceutical budget enforced by rebates (volume-based returns) and clawback (collective repayment of spending above the cap). Both are high and directly erode realised revenue — so the headline bulletin price rarely reflects what a company actually keeps.

Recent developments

2025 and 2026 have brought additional rebates on high-cost reimbursed medicines and a mechanism allowing companies to offset part of their clawback against research and investment spending. The framework remains a moving target, and a launch plan built on last year’s assumptions can quickly become inaccurate.
